As part of the therapeutic process, it is important that the responsibilities and limitations by your therapist, the practice and yourself are clearly outlined and agreed upon to ensure a safe and positive experience for all involved.

By making an online appointment, it indicates that you agree to the following:

The psychological services provided may include but are not limited to individual therapy, family therapy, couples therapy, and psychological assessments.

 

These services will be conducted by qualified mental health professionals in accordance with their respective professional licenses and ethical standards.

C O L L E C T I N G   &   S T O R I N G   I N F O R M A T I O N

In order to provide effective psychological services, it is necessary for the therapist to collect and store personal information about clients. This information may include, but is not limited to, demographic information, medical history, mental health history, and session notes.

 

You consent to the collection and storage of this information in accordance with applicable laws and regulations.

C O N F I D E N T I A L I T Y

All information disclosed during therapy sessions is confidential and will not be disclosed to any third party without written consent, except where required by law. You understand that confidentiality may be breached in the following circumstances:

 

  • If there is an imminent risk of harm to the client or others.

 

  • If there is suspected abuse or neglect of a child, elderly person, or dependent adult.

 

  • If clients disclose information about the commission of a crime.

 

  • If disclosure is required by a court order.

A C C E S S   T O   C L I E N T   I N F O R M A T I O N

Understand you have the right to access your personal information and request copies of your records in accordance with applicable laws and regulations.

 

You may request access to your information by submitting a written request to your therapist at Atypical Minds, where they will discuss appropriate forms of access.

C A N C E L L A T I O N   P O L I C Y

Appointments must be canceled at least 24 hours in advance. Failure to provide 24 hours’ notice may result in a cancellation fee.

 

Extenuating circumstances may be considered, and exceptions may be made at the discretion of your therapist.

You acknowledge that you have contacted Atypical Minds to ask questions and have received satisfactory answers regarding the nature and purpose of the psychological services offered, as well as the policies outlined.
